Documents to bring:
- A copy of your current physical, dated within the past year.
- Supplemental Medical Form, if you did not turn it in with your digital packet.
If you HAVE NOT filled out the digital packet, you will NOT be able to participate until it is completed.
Open to: Eligibility: Grade 9-12 Required Documents: Current Yearly Physical on file with the nurse and Required Athletics Documents Completed; must maintain a 2.0 GPA or above with no more than 1 F at any time; 10 practice dates before first competition
